Esophageal Stricture due to Corrosive Agents



Abstract
A clinical study on 190 cases of benign esophageal stricture caused by ingestion of lye and other corrosive agents, who were admitted to Severance Hospital during the period from. Jan. 1953 to Aug. 1972 was analyzed and presented.
The diagnosis was made by clinical examination and other laboratory studies and. was confirmed mainly by the X-ray studies.
1. The age of the patients ranged from 1 to 63 years and the majority of patients were between 20x30 years of age (43. 7%).
2. The ratio of sex of female to male was 2 : 1.
3. The levels of strictures are as follow; hypopharynx 8 caess, cervical esophagus 48 cases upper, thoracic esophagus 64 cases, midthoracic esophagus 29 cases, lower thoracic esophagus 14 cases, multiple levels 16 cases and uncertain 11 cases.
4. In 190 cases of esophageal stricture, 56 cases were undertaken gastrostomy only 28 cases.bougination only, 33 cases conservative treatments, 65 cases substernal esophagoplasty .and other procedures in 8 cases.
5. A preliminary gastrostomy was performed two to four months prior to substernal esophagoplasty in 27 cases.
6. The utilized segments of bowel were right colon in 10 cases, right colon and terminal ileum in 53 cases, left colon in 1 case and jejunum in 1 case.
7. There were total 61 complications in 24 cases and the majority of them were pneumothorax 18 cases, wound infections 13 cases, leakage from esophagocolic anastomosis 7 cases, stenosis in cervical anastomosis 9 cases, bowel necrosis 5 cases and others¢¥ 11 cases.
8. There were no operative mortalities and seven patients were died because of secondary problems such as pulmonary insufficiency, panperitonitis, excessive bleeding etc. rather than of esophageal stricture itself.
9. In succesful cases of esophagoplasty, swallowing and passage of foods were satisfactory two to four weeks after operation.
